Health Promotion Services

Immunization Clinic

• Childhood immunizations

• Adult immunizations and influenza injections

• Appointments are necessary

H.O.P.E.S (Healthy Opportunities for Parents to Experience Success)

• In home visits by agency staff

• Assistance with local resources

• Linkage of family to medical providers

• Serve families with children prenatal to 5 years of age

Stork's Nest Program (Promoting Healthy Mothers and Healthy Babies

• Incentive program

• Earn free baby items

• Points are earned by normal pregnancy visits and healthy behaviors

• Educational parenting classes offered

• Participants must be pregnant (must be enrolled by the 7th month)

Medication Assistance Program

• Our goal is to keep good medicine within your reach

• Prescription Savings Program for eligible recipients

Emergency Preparedness

• Bio-Emergency Response Plan

• Disease Surveillance

Resource and Referral

• Knowledgeable of community resources available
